W-8 / W-9 Forms

Please read the section below that pertains to you, and return the linked form as indicated:

  Non-American Travel Grant Recipient

All non-American individuals receiving money from MDS for work done outside of the United States are required to complete a W-8 form for the Society to keep on file.

This form is simply to state that the recipients are not citizens of the United States, and unless the recipients were to become citizens of the United States, this form would only need to be completed once. The Society is not required to withhold tax on payments to non-American individuals and entities for work done outside of the USA and the W-8 form provides the Society with proof of an individual’s citizenship. Payment cannot be issued if MDS does not have a copy of your W-8 on file.

W-8 Form

  American Travel Grant Recipient

All American individuals receiving money from the Society must complete a W-9 form. Payment cannot be issued if MDS does not have a copy of your W-9 on file.

W-9 Form

  Dual Citizenship (American)Travel Grant Recipient

If you have dual citizenship (American citizenship plus any other country), please complete the W-9 form.
